Voyage Specialist Healthcare is on a Journey of Change towards Personalisation and Co–Production

The Journey to delivering person centred care

Voyage Specialist Healthcare are committed to developing Person Centred care and ways of thinking which will allow us to understand the needs of our clients. Our goal is to provide a service which supports the life which our clients wish to lead.

Working in partnership with our clients and their families to co-produce their care plans is central to the success of our company. We are devoted to working better with our clients and the wider community and are focused on developing the skills which will allow our staff to plan and solve problems with our clients.

What has happened so far?

In Sept 2012 Voyage Specialist Healthcare appointed a Family Support Worker to improve client engagement and to support the organisations ability to deliver person centred care. Our family support worker has been instrumental in facilitating the changes which have already been made on our journey to delivering person centred care for all of our clients.

In November 2012, we developed a working relationship with Helen Sanderson Associates and have initiated a joint piece of work which will utilise person centred tools to support clients with complex physical health care needs.

A client engagement tool was completed between October 2012 and Jan 2013 and highlighted that our clients are looking for a service which is Person Centred, Reliable, Responsive and does not compromise on Quality. This tool highlighted that our clients required additional improvements and flexibility in the times which their care hours were delivered. Based on these comments we have revised our rota management policy and introduced a rota management workshop which has been attended by all of our staff. Our approach to personalising care has also been formalised through re-writing our care plans and providing clients with a welcome pack. Our care plans and welcome pack are now person centred in nature.

We are committed to the Making it Real Agenda which seeks to support organisations in the delivery of personalised care www.thinklocalactpersonal.org.uk. Feedback from our monthly and annual client satisfaction audits have helped to establish the following three areas of progress which will help to develop our approach to delivering person centred care:

We learnt that whilst our staff worked in a person centred way, we did not formalise this approach. We have changed our care assessment documentation and care plans to include and formalise person centred approaches. We will allow time for the organisation to become familiar with the new documentation and review the impact and effectiveness with our clients and staff. Sept 2013
We could improve our approach to matching the interests and personalities of our clients and staff. We have introduced a “matching tool” from Helen Sanderson Associates into our assessment documents. We will develop one page profiles for our carers, to support the matching process. Template to be ready by May 2013, ongoing roll out.
Our approach to delivering person centred care has been introduced in a very short timeframe. To embed this change there is need to allow our staff to fully adopt and understand the changes. The changes to have been mainly initiated by the management team. We will further engage our staff and clients to develop a person centred care training session. This training session will be delivered by clients and our staff and emphasise the changes we have already made. First training session to be available May 2013, with a plan of on-going review and change
